Position Overview

We are currently seeking Licensed Practical Nurses (LPNs) to join our team and deliver high-quality in-home care to clients in Fairfax, Virginia. This flexible role offers assignments based on client needs and availability. The ideal candidate will possess strong clinical skills, a dedication to compassionate care, and the ability to work independently in a home care setting.

Key Responsibilities

• Patient Care: Provide personalized care, including medication administration, monitoring vital signs, and assisting with daily activities.
• Documentation: Ensure accurate and timely documentation of patient interactions and medical records in line with healthcare standards.
• Collaboration: Work closely with registered nurses, physicians, and family members to ensure comprehensive care plans.
• Patient and Family Education: Offer guidance on health management, treatment plans, and recovery processes.
• Compliance and Safety: Follow healthcare policies and safety protocols to maintain a safe and effective care environment.

Home Care Services Provided

• In-home medical assistance
• Senior care services

• 24-hour clinical support access

Requirements

• Licensure: Active and unrestricted LPN license in Virginia.
• Experience: Previous home care or similar healthcare experience preferred.
• Education: Completion of an accredited LPN program.

• Communication: Strong interpersonal skills with the ability to effectively educate and interact with patients and their families.
