Registered Nurse – Home Care (Pediatric and Teenage Patients)

We are looking for a compassionate and skilled Registered Nurse to provide high-quality medical and personal care to pediatric and teenage patients in their homes. The ideal candidate will have a strong nursing background and the ability to deliver individualized care to young patients with complex medical needs, including tracheotomy management and full support for daily living activities. This role plays a crucial part in ensuring the health, comfort, and well-being of both patients and their families.

Key Responsibilities:

• Conduct thorough patient assessments, including monitoring vital signs and reviewing medical history to evaluate health needs and track progress.
• Develop and implement personalized care plans tailored to the unique needs of pediatric and teenage patients.
• Administer prescribed medications and treatments, ensuring adherence to prescribed regimens.
• Provide hands-on support with daily living activities such as bathing, feeding, and maintaining personal hygiene.
• Care for patients with tracheotomies, including site cleaning, suctioning, and addressing complications as needed.
• Observe patients closely, documenting any changes or concerns and promptly reporting them to healthcare professionals.
• Collaborate with an interdisciplinary care team to ensure effective, comprehensive patient care.
• Educate patients’ families on health management, tracheotomy care, and other aspects of caregiving.
• Maintain accurate medical records that comply with health regulations and standards.
• Follow infection control protocols and safety guidelines to ensure a safe environment for patients.

Qualifications:

• Licensure: Valid nursing license in the state of practice or multi-state Compact license.
• Experience: Prior experience in home care, pediatric care, or similar settings is preferred.

Skills:

• Proficiency in monitoring vital signs and administering treatments.
• Expertise in tracheotomy care and managing medically fragile patients.
• Strong documentation skills for maintaining patient health records.
• Exceptional communication skills for interacting with patients, families, and healthcare teams.

Additional Requirements:

Commute Flexibility: Willingness to commute up to 40 minutes from home to patient locations for timely and reliable care delivery.

Weekend Availability: Willing to work 4 out of 8 weekend days per month (e.g., alternating weekends, every Saturday or Sunday for day or night shifts).

Transportation Requirements: Must have a valid driver’s license and a reliable vehicle. Flexibility to care for multiple patients during shifts if needed. Travel expenses from the first to the last patient are reimbursed.

Client Adaptability: Comfort working with a diverse range of patients, including pediatric and geriatric populations, based on assignment needs.

Start Date Readiness: Available to begin work within 3 to 6 weeks of hire to meet patient care demands.

Salary:

$39 – $41 per hour (dependent on experience)
