Location: 3040 W Salt Creek Lane, Arlington Heights, IL
Schedule: Full-Time, 40 hours/week
Hours: Monday – Friday, 8:30 AM – 5:00 PM
Hourly Pay: $40.45 – $62.70 (based on experience and expertise)
Position Summary
The RN Transitional Care Navigator plays a vital role in population health management by overseeing case management, care coordination, and utilization management across various care levels and settings. This role is dedicated to ensuring seamless care transitions, reducing healthcare costs, and improving overall patient outcomes through collaborative and evidence-based care.
You will act as a bridge between patients and providers, empowering individuals to understand their diagnoses, treatment plans, and available resources throughout their care journey.
Key Responsibilities
- Serve as the primary care coordinator for a panel of high-risk patients.
- Facilitate care transitions and ensure appropriate resource utilization across the continuum.
- Develop individualized, evidence-based care plans focusing on wellness, prevention, and chronic disease management.
- Collaborate with interdisciplinary teams to support discharge planning and clinical decision-making.
- Support patients and families with education on medication management, community resources, and financial support.
- Guide patients through consultations, testing, and follow-up care.
- Perform utilization reviews and apply Milliman Care Guidelines (Indicia).
- Monitor length of stay (LOS), ancillary services, and cost-of-care metrics.
- Occasionally conduct home visits to ensure continuity and quality of care.
Required Qualifications
- Education: Bachelor’s degree in healthcare or related field required; BSN from an NLN-accredited nursing program preferred.
- License: Active Registered Nurse (RN) license required.
- Certifications: BLS/CPR required; case management or ambulatory care certification preferred.
Experience:
- Minimum 3 years in utilization review, discharge planning, case/disease management.
- Home services or ambulatory care experience with high-risk populations is a plus.
- At least 2 years of clinical nursing experience preferred.
