Key Responsibilities:

• Perform comprehensive patient assessments, including monitoring vital signs and reviewing medical histories to evaluate health needs.
• Develop and implement personalized care plans based on each patient’s unique medical needs.
• Administer prescribed medications and treatments in accordance with physician guidelines.
• Assist with daily living activities such as bathing, feeding, and hygiene support.
• Provide tracheotomy care, including cleaning, suctioning, and addressing potential complications.
• Track patient progress, report any changes in condition to the healthcare team, and adjust care plans as needed.
• Collaborate with interdisciplinary teams to ensure coordinated and holistic patient care.
• Educate patients’ families on health management, tracheotomy care, and other important caregiving tasks.
• Maintain accurate, up-to-date patient records in compliance with health and safety regulations.
• Ensure a safe and clean environment for patients by adhering to infection control protocols and safety standards.

Qualifications:

• Licensure: Valid RN license in the state of New Jersey (or multi-state Compact license).
• Experience: Prior experience in pediatric or home care is preferred, but not required.

Skills:

• Proficient in administering medications and monitoring vital signs.
• Expertise in tracheotomy care and managing medically fragile patients.
• Strong documentation skills and knowledge of health record compliance.
• Excellent communication skills for interaction with patients, families, and healthcare teams.

Additional Requirements:

Commute Flexibility: Willing to travel up to 40 minutes between patient locations.

Weekend Availability: Must be available to work 4 out of 8 weekends per month (rotating schedule for day or night shifts).

Transportation: Must have a valid driver’s license and reliable transportation. Travel reimbursement is provided for patient visits.

Patient Demographics: Comfortable working with diverse patient populations, including pediatric, geriatric, or male/female patients.

Start Date: Ability to begin within 3 to 6 weeks of hiring.
